After your posting for a student position in the OSU Jobs Online Recruiting System has closed, please submit this form to notify FOBC HR of the applicant that you wish to hire.  Required fields are marked with asterisks (*).

Once you submit the form, an FOBC HR team member will check to see if the student needs to complete any employment paperwork.  If so, we will notify the student and supervisor via email.  It is recommended that students do not begin work until they complete their employment paperwork.  FOBC HR will notify the supervisor via email when employment paperwork has been completed.

IMPORTANT NOTE:  If the student position requires a Criminal History Check (CHC) or Motor Vehicle History Check (MVC), the student may not begin working until their CHC/MVC has been cleared by the Office of Human Resources (OHR).  OHR will send a Satisfactory Notice to FOBC HR and the supervisor via email when the CHC/MVC is complete.

For questions regarding this form, or for questions regarding student employment, please contact FOBC Human Resources at fobc.hr@oregonstate.edu

POSITION INFOMRATION

Becuase you hired from a pool, there is additional information that you will need to provide in order for FOBC HR to determine whether this position requires a Criminal History Check (CHC) and/or Motor Vehicle History Check (MVC).  If a CHC and/or MVC is required, you will be notified by FOBC HR, and the student may not begin work until their CHC and/or MVC has been cleared by the Office of Human Resources (OHR).  OHR will send a Satisfactory Notice to FOBC HR and the supervisor via email when the CHC and/or MVC is complete.
